Roles & Responsibilities

• Provide skilled physical therapy in a home health setting to help patients regain independence
• Perform thorough evaluations and create individualized treatment plans focused on meaningful outcomes
• Deliver hands-on therapy and teach safe mobility, transfer and exercise techniques
• Document patient care clearly and timely in electronic health records (Epic preferred)
• Complete Trinity specialty competency testing and maintain required skills checklists
• Maintain required certifications and health screenings so patients and your team stay safe
• Collaborate with interdisciplinary team members and patients' families to coordinate care
• Manage caseload effectively to meet patient needs and assignment schedule

Qualifications

• Valid Physical Therapy licensure (able to verify license/certification)
• BLS certification required
• Experience or comfort working in Home Health settings (Home Health listed as a core skill)
• Completion of Trinity Skills/Competency Testing and Trinity Specialty Specific Competency Testing as required
• Epic experience preferred but not required
• Ability to meet personal health and immunization requirements listed in the onboarding checklist

Knowledge & Skills

• Strong clinical assessment and home-based treatment planning skills
• Skilled in patient education, fall prevention and functional mobility training
• Comfortable with documentation workflows and electronic health records (Epic preferred)
• Familiarity with infection control practices and routine immunization/compliance requirements
• Excellent communication and teamwork skills for coordinating home care
• Attention to detail for completing competency checklists and maintaining licensure
