Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to restore function and independence
• Develop and implement individualized treatment plans that target residents' goals
• Collaborate closely with an interdisciplinary team to ensure coordinated, holistic care
• Document thoroughly and timely to meet facility and regulatory standards
• Participate in care conferences and contribute to quality improvement initiatives
• Educate residents and families to support safe discharge planning and ongoing recovery

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical reasoning and hands-on physical therapy skills
• Excellent communication skills for working with residents, families, and interdisciplinary teams
• Skilled nursing or post-acute experience preferred
• Dependable, flexible, and team-oriented approach to patient care
• Commitment to delivering high-quality, compassionate therapy

Knowledge & Skills

• Physical therapy evaluation and treatment planning for post-acute and skilled nursing populations
• Accurate clinical documentation and familiarity with regulatory compliance
• Interdisciplinary team collaboration and care conference participation
• Patient-centered care and rehabilitation goal setting
• Time management and prioritization in a busy rehab environment
• Strong interpersonal skills and family/resident education techniques
