Roles & Responsibilities
- Establish collaborative practice programs with oncologists to optimize inpatient and outpatient supportive care
- Lead staff development and educational sessions for pharmacy, nursing, and medical teams
- Serve as the oncology drug information expert and ensure EHR care plans align with national guidelines
- Round with Fellows, Nurse Practitioners, and Physicians to recommend therapy adjustments and monitoring
- Provide patient-centered medication education using teach-back for oral oncolytics and IV chemotherapy
- Perform ongoing reviews and updates of oncology care plans within the Electronic Health Record
- Identify and resolve medication-related problems using clinical data and patient history
- Precept pharmacy residents and students, supporting the next generation of oncology clinicians
- Participate in Oncology Service Line meetings to improve safety, quality, and outcomes
- Promote rational, cost-effective drug therapy and focus on cost avoidance and savings within the service line
- Perform other duties as assigned within the scope of the role to support excellent cancer care
Qualifications
- Doctor of Pharmacy (PharmD) required
- Active Pharmacist licensure required (current and in good standing)
- Board Certified Oncology Pharmacist (BCOP) strongly preferred or expected
- Completion of PGY-1 residency with clinical pharmacy practice experience or PGY-2 oncology residency or equivalent experience
- Minimum of three years of hospital clinical pharmacy experience with at least three years in oncology practice
- Preferred: PGY-2 ASHP-accredited residency or PGY-1 plus ≥3 years related experience and BPS oncology certification; candidates with ≥5 years plus BPS oncology certification are highly valued
Education
- Doctor of Pharmacy (PharmD)
- PGY-1 residency (or equivalent clinical pharmacy experience) required; PGY-2 oncology residency preferred
Knowledge & Skills
- Expertise in oncology pharmacy practices, including IV chemotherapy and oral oncolytics
- Proficiency with Electronic Health Record review and oncology care plan optimization
- Strong patient education skills using teach-back and supportive care counseling
- Experience with collaborative practice, interdisciplinary rounding, and clinical decision-making
- Capability to provide drug information and evidence-based recommendations aligned with national guidelines
- Precepting and teaching skills for residents and students
- Focus on medication safety, cost-effective therapy, and quality improvement
