Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM Day; Weekend rotation every other weekend required
Hours per Week: 40
Candidate Pay Rate: $75 per hour

Roles & Responsibilities
- Coordinate comprehensive care plans and perform discharge planning for adult and geriatric patients
- Conduct needs assessments and independently arrange DME, SNF placement, and ambulance transport
- Facilitate and participate in family meetings and contribute to treatment plan development
- Document thoroughly and accurately in Epic to ensure continuity of care
- Manage worker's comp, disability case management, appeals and denials processes when needed
- Ensure care coordination across HMOs/IPAs and community resources for safe transitions
- Apply regulatory knowledge (CMS, DRG, HIPAA, OSHA, The Joint Commission) to daily practice
- Work collaboratively with interdisciplinary teams to optimize patient outcomes and throughput
Qualifications
- California state license required (pending license accepted but must be active by start date)
- Minimum 2 years acute medical Care Manager experience performing care coordination
- Travel experience required — first-time travelers will not be considered for this role
- Proven proficiency with Epic or similar electronic charting systems (Epic required)
- Comfort working with patient ratios up to 1:25 in an acute care setting
- Weekend rotation availability (every other weekend) and RTO restrictions (no more than 7 days)
- Experience with appeals/denials, worker's comp case management, and arranging post-acute care
Knowledge & Skills
- Care coordination and case management in an acute hospital environment
- Discharge planning and transition of care best practices
- Epic electronic medical record documentation and workflows
- Regulatory familiarity: CMS, DRG, HIPAA, OSHA, The Joint Commission
- Needs assessment, Plan of Care development, and ordering DME
- Experience with Emergency Department, ICU, Medical-Surgical, and Surgical Services (preferred)
- Appeals and denials processes and disability case management
- Strong communication skills for family meetings and interdisciplinary collaboration
