Roles & Responsibilities

• Provide compassionate, resident-centered nursing care for long-term care patients
• Administer medications and treatments accurately and safely during your shift
• Document care and clinical observations using PCC (PointClickCare) thoroughly
• Collaborate with RNs and interdisciplinary team members to support resident care plans
• Perform routine assessments, report changes in condition, and escalate concerns promptly
• Support a safe, clean environment and adhere to infection control practices
• Float between two floors as needed to meet resident care demands with flexibility

Qualifications

• Current LPN license in good standing for the state of Wyoming
• Experience in long-term care (LTC) settings preferred
• Familiarity with PCC (PointClickCare) charting is a plus
• Current CPR/BLS certification
• Comfortable working 12-hour day shifts and able to float between units
• Strong communication and teamwork skills, with a resident-first mindset

Education

• Practical Nursing diploma or equivalent (LPN)

Knowledge & Skills

• Medication administration and documentation
• Basic wound care and ADL support for long-term care residents
• Resident assessment, care planning, and clinical observation
• Proficiency with PCC electronic health records
• Infection control standards and safe patient handling
• Effective communication with residents, families, and clinical team
