Roles & Responsibilities

• Facilitate patient transfers across Dignity and external hospitals, securing accepting physicians and appropriate placements
• Perform transfer-related patient assessments and document vitals and clinical reports accurately
• Coordinate bed placement requests and interact with bed management systems (TeleTracking)
• Communicate clearly and promptly with physicians, specialists, and receiving facilities using TigerConnect and other channels
• Chart encounters and transfer details in the EMR (Cerner and/or EPIC) with attention to detail
• Manage multiple simultaneous cases efficiently — average caseload ~12 per shift, sometimes 10–15 at once
• For LVNs: prioritize refer-outs when internal placement is not available
• Participate in orientation and on-shift training to maintain competency and unit readiness

Qualifications

• Active RN or LVN license required (must be in good standing)
• Minimum 2 years of ICU, ED, or transfer center experience (posting also references a 3-year experience field in some sections)
• Acute care experience required with strong critical care assessment skills and clinical judgment
• Required certifications: ACLS (AHA), BLS (AHA), NIHSS
• Experience with Cerner or EPIC and TeleTracking strongly required; comfortable with multiple clinical applications and Google Workspace
• Strong customer service skills, multitasking ability, and solid computer/typing skills (typing test recommended)
• Relias Dys exam required prior to acceptance (must be completed within 24-48 hours of acceptance unless a valid Relias Dys from within 12 months is provided)
• Candidates may not have been directly employed by CommonSpirit/CHI/Dignity within the past year (PRN via outside firms may be considered)
• Professional demeanor and ability to learn quickly in a high-volume environment

Education

• Active RN or LVN license required
• BSN not required
