Roles & Responsibilities

• Provide compassionate direct patient care and assist residents with daily needs
• Receive and give clear reports on resident condition to ensure safe continuity of care
• Make rounds, assess new residents and quickly identify changes in condition
• Document assessments and care per facility policy with accuracy
• Notify nurses and care team of any changes or new needs promptly
• Respond to call lights promptly and ensure resident comfort and safety
• Promote positive working relationships with residents, families, physicians and other departments
• Float to other units as needed and perform other duties as assigned

Qualifications

• California CNA License (MUST have) — active and in good standing
• Current CNA certification with experience in skilled nursing or long-term care
• Strong communication skills and proven ability to work as a team player
• Comfortable floating to other units and adapting to different teams
• Willingness to work weekends, holidays and rotating shifts

Knowledge & Skills

• Resident assessment and recognizing changes in condition
• Accurate documentation and reporting per facility policies
• Direct patient care including activities of daily living assistance
• Prompt response to call lights and resident needs
• Effective communication with residents, families and interdisciplinary teams
• Teamwork and collaboration across departments
