Roles & Responsibilities

• Assess patients' functional abilities and develop individualized occupational therapy plans
• Implement therapeutic interventions to improve independence with ADLs and IADLs
• Collaborate with the interdisciplinary team to coordinate patient-centered care
• Document progress and treatment outcomes in the facility's documentation system
• Provide caregiver and family education to support carryover of therapy goals
• Monitor and adjust treatment plans based on patient response and changing needs
• Participate in discharge planning and ensure safe transitions of care

Qualifications

• Licensed Occupational Therapist (OT) in Oklahoma or eligible to obtain Oklahoma licensure
• NBCOT certification preferred but not required
• Open to clinicians of all experience levels, including new graduates
• Ability to complete facility onboarding requirements (background check, drug screen, immunizations) as required
• Strong clinical reasoning and patient-centered approach appropriate for a SNF setting

Education

• Degree in Occupational Therapy (Bachelor's/Master's/OTD) meeting state licensure requirements
• New grads welcome

Knowledge & Skills

• Skilled in functional assessments, ADL retraining, and therapeutic interventions in a SNF environment
• Proficient documentation and care planning skills
• Comfortable working as part of an interdisciplinary rehab team
• Good communication skills with patients, families, and staff
• Time management and caseload prioritization in a fast-paced setting
