Roles & Responsibilities

• Provide compassionate, hands-on personal care to residents — bathing, dressing, grooming and toileting assistance
• Support the licensed nursing team by monitoring and reporting changes in resident condition
• Perform routine vital signs and document findings accurately in the care record
• Assist with mobility and safe transfers to promote resident independence and prevent falls
• Follow individualized care plans and contribute to care plan updates through clear communication
• Maintain a clean, safe environment and adhere to infection control policies
• Communicate respectfully with residents and families, fostering dignity and trust
• Collaborate with interdisciplinary team members to ensure high-quality, person-centered care

Qualifications

• Active Licensed Nursing Assistant (LNA) certification — VT or NH licensure indicated by job title
• Current basic life support (BLS/CPR) certification preferred or willing to obtain
• Ability to pass required background checks and health screening (immunizations, TB screening)
• Comfortable working 12+ hour day shifts and participating in a rotating weekend schedule
• Strong communication skills and a team-oriented, patient-first mindset

Education

• High school diploma or equivalent
• Completion of an approved LNA training program and current LNA credential

Knowledge & Skills

• Fundamental resident care and ADL assistance
• Accurate vital signs measurement and documentation
• Basic infection prevention and safety practices
• Good time management and ability to prioritize under busy conditions
• Clear, compassionate verbal communication with residents and families
• Team collaboration within a long-term care/rehabilitation setting
