Roles & Responsibilities

• Evaluate residents to determine communication and swallowing needs and set individualized goals
• Develop and implement evidence-based treatment plans to improve speech, language, and swallowing function
• Document progress clearly and timely in the clinical record, supporting continuity of care
• Collaborate with nursing, PT/OT, dietary, and family members to coordinate comprehensive care
• Provide patient- and family-centered education to promote carryover of strategies and safety
• Participate in interdisciplinary team meetings and discharge planning to optimize outcomes
• Advocate for residents’ functional communication needs and contribute to quality improvement initiatives

Qualifications

• ASHA Certificate of Clinical Competence (CCC-SLP) indicated on the order — bring your credential
• Licensed in New Hampshire or eligible for NH licensure (licensure assistance available)
• Comfortable working in a skilled nursing facility (SNF) setting; experience with dysphagia and geriatric populations a plus
• Open to all experience levels — including new grads (posting explicitly welcomes new graduates)
• Strong clinical documentation skills and ability to work collaboratively within an interdisciplinary team

Knowledge & Skills

• Clinical assessment and treatment of speech, language, cognition, and swallowing
• Experience in SNF/regulatory documentation and care planning
• Interdisciplinary collaboration and effective clinical communication
• Skilled in developing individualized, functional goals and measurable outcomes
• Ability to educate residents, families, and care teams on strategies and safety
