Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications accurately and document medication administration per policy
Monitor and record vital signs and changes in resident condition promptly
Collaborate with nurses, therapists, and the interdisciplinary team to deliver coordinated care
Assist with activities of daily living, admissions, transfers, and care planning
Maintain clear, professional documentation in the resident record
Adhere to infection control standards and facility policies to ensure safety

Qualifications

Current LPN (Licensed Practical Nurse) license in Pennsylvania
Completion of an accredited practical nursing program
BLS/CPR certification or willingness to obtain upon hire
Ability to work 8.5-hour shifts as scheduled and provide dependable attendance
Strong communication skills and a compassionate approach to resident care
Comfortable working in a long-term or skilled nursing environment

Education

Graduate of an accredited Practical Nursing program
High school diploma or GED

Knowledge & Skills

Medication administration and basic pharmacology knowledge
Resident assessment and documentation skills
Familiarity with infection control practices and long-term care standards
Basic wound care and restorative nursing support
Comfort using electronic medical record systems
Team collaboration and effective communication with residents and families
