Roles & Responsibilities

• Evaluate residents’ speech, language, cognitive-communication and swallowing function using evidence-based assessments
• Develop and implement individualized treatment plans focused on meaningful functional outcomes
• Collaborate with nursing, therapy colleagues, and families to create coordinated care plans
• Document progress and outcomes clearly and on time to support continuity of care
• Provide education and training to caregivers and staff to enhance patient carryover
• Participate in interdisciplinary meetings and contribute to quality improvement initiatives

Qualifications

• Licensed Speech-Language Pathologist (SLP) in California (required or eligible)
• Experience working in a Skilled Nursing Facility or post-acute setting preferred
• Strong background in dysphagia management and cognitive-communication disorders
• Excellent documentation and communication skills
• Comfortable working autonomously while collaborating within an interdisciplinary team

Education

• Master’s degree in Speech-Language Pathology (or clinically equivalent degree)

Knowledge & Skills

• Proficient with evaluations and treatment for adult speech, language, cognition and swallowing
• Knowledge of Medicare/Medicaid documentation and SNF clinical processes
• Skilled in patient-centered goal writing and outcome measurement
• Ability to educate caregivers and staff in practical carryover strategies
• Familiarity with electronic medical records and timely clinical documentation
