Roles & Responsibilities

• Evaluate patients thoroughly to identify functional limitations and rehabilitation goals
• Develop and implement individualized treatment plans based on best practices
• Deliver skilled physical therapy interventions including manual therapy and therapeutic exercise
• Educate patients and families on home programs, safety, and progress expectations
• Document assessments, treatment plans, and progress notes clearly and timely in the medical record
• Collaborate with physicians, nurses, and the interdisciplinary team to optimize patient outcomes
• Participate in quality improvement initiatives and help maintain a high standard of care
• Maintain professional development and stay current with evidence-based practice

Qualifications

• Current Illinois Physical Therapy license (required)
• Doctor of Physical Therapy (DPT) preferred; bachelor’s or related degree with equivalent experience considered
• Basic Life Support (BLS) or CPR certification (or willingness to obtain)
• Previous clinical experience in outpatient, acute, or rehabilitation settings preferred
• Strong communication skills and ability to work collaboratively with interdisciplinary teams
• Willingness to complete required onboarding and credentialing for a travel contract

Education

• Doctor of Physical Therapy (DPT) preferred; or Bachelor’s degree in Physical Therapy with valid state licensure

Knowledge & Skills

• Skilled in assessment techniques, therapeutic exercise, and manual therapy
• Experience with gait training, balance interventions, and functional mobility training
• Comfortable using electronic medical records and documenting according to facility standards
• Good clinical reasoning and problem-solving skills
• Patient-centered care approach with strong interpersonal and teaching skills
