Roles & Responsibilities

• Assess residents’ functional abilities and needs using evidence-based OT approaches
• Develop and implement individualized treatment plans that improve daily living and independence
• Provide therapeutic interventions for ADLs, cognitive and mobility challenges in a skilled nursing setting
• Collaborate closely with nursing, PT, SLP and interdisciplinary teams to coordinate care
• Document progress thoroughly and communicate outcomes to residents, families, and the care team
• Educate residents and families on adaptive techniques, equipment, and home safety
• Support discharge planning and continuity of care with clear, resident-centered goals

Qualifications

• Licensed Occupational Therapist in California or eligible to obtain state licensure
• Open to all experience levels — any experience accepted, including new grads
• Strong clinical assessment and treatment planning skills specific to SNF populations
• Excellent communication and teamwork skills for interdisciplinary collaboration
• Current CPR/Basic Life Support (BLS) certification as required by facility policies

Education

• Degree in Occupational Therapy (entry-level OT degree such as BS/OT or Master's/OTR preferred) or graduating candidate eligible for licensure

Knowledge & Skills

• Skilled in ADL training, adaptive equipment recommendations, and therapeutic modalities
• Experience or comfort working in a Skilled Nursing Facility (SNF) environment
• Strong documentation skills and familiarity with clinical charting best practices
• Ability to assess functional mobility, cognition, and safety for older adults
• Person-centered approach with excellent interpersonal and coaching skills
