Roles & Responsibilities

Assist licensed therapists in delivering patient-centered rehabilitation interventions
Help implement treatment plans and carry out therapeutic activities to improve patient function
Monitor and document patient progress clearly and promptly in clinical records
Collaborate with nurses, therapists, and the interdisciplinary team to coordinate care
Support safe patient mobility and transfer techniques during daily therapy sessions
Engage patients and families with education and encouragement to meet functional goals

Qualifications

Current or eligible Physical Therapist Assistant (PTA) licensure (state-specific requirements may apply)
New grads welcome — open to any level of experience, including new graduates
Comfortable working in a skilled nursing facility (SNF) setting
Ability to float between units as needed and adapt to varying caseloads
Strong documentation and communication skills

Knowledge & Skills

Understanding of SNF workflows and rehab-focused care
Familiarity with therapeutic modalities and mobility training
Skilled in safe patient handling and transfer techniques
Competent clinical documentation and progress note practices
Team collaboration and time management in a fast-paced environment
