Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and safely
Monitor and document vital signs, symptoms, and patient changes
Collaborate with RNs and interdisciplinary team members to coordinate care
Maintain clear, timely documentation in the patient record
Assist with activities of daily living and support patient comfort
Participate in care planning and communicate observations to the care team
Help create a welcoming, respectful environment for patients and families

Qualifications

Current LPN license in Pennsylvania or eligibility to obtain licensure
Basic Life Support (BLS/CPR) certification preferred or required by facility
Strong clinical skills in medication administration and patient assessment
Excellent communication and teamwork skills
Ability to pass required background and health screenings

Education

Graduate of an accredited practical nursing program or equivalent credential

Knowledge & Skills

Knowledge of basic nursing principles and medication administration
Familiarity with infection control and patient safety practices
Comfortable with charting and electronic health records
Strong observational skills and attention to detail
Compassionate patient-centered approach and good time management
