Roles & Responsibilities

• Provide skilled physical therapy assessment and hands-on treatment to residents in a skilled nursing setting
• Develop individualized treatment plans that promote function, mobility, and independence
• Perform gait, balance, transfer, and ADL training with a focus on safety and measurable outcomes
• Document timely, accurate clinical notes and progress reports compliant with facility and payer requirements
• Collaborate closely with nursing, OT, SLP, and case management to coordinate comprehensive care
• Educate patients, families, and staff on mobility strategies, fall prevention, and safe transfer techniques
• Participate in care conferences and contribute to discharge planning and continuity of care

Qualifications

• Doctor of Physical Therapy (DPT) — listed as a core skill for this role
• Eligible for or holding an active Physical Therapist license in Florida
• Open to all experience levels, including new graduates (years of experience: any, including new grad)
• Comfort working in a Skilled Nursing Facility (SNF) setting; SNF experience is a plus
• Strong communication and interdisciplinary collaboration skills
