Roles & Responsibilities

• Assist licensed physical therapists in delivering skilled therapy interventions in a skilled nursing facility
• Implement treatment plans and carry out therapeutic exercises to improve patient mobility and function
• Monitor and document patient progress clearly and compassionately
• Support safe patient transfers, gait training, and use of assistive devices
• Collaborate with nurses, therapists, and the care team to coordinate excellent patient-centered care
• Educate patients and families on exercises, safety, and home programs to support recovery

Qualifications

• Primary skill: PTA (Physical Therapist Assistant) — credentialed or eligible per state requirements
• Comfortable working in a Skilled Nursing Facility (SNF) practice setting
• Years of experience: any, including new graduates — enthusiasm and willingness to learn welcomed
• Ability to pass facility onboarding requirements (background check, immunizations) as needed

Knowledge & Skills

• Foundational PTA therapy skills and knowledge of SNF care
• Patient handling and safe mobility techniques
• Ability to follow treatment plans and document clinical progress
• Strong communication and teamwork with interdisciplinary staff
