Roles & Responsibilities

• Provide compassionate, high-quality nursing care in an outpatient clinic setting
• Triage patients and perform focused assessments to support providers
• Administer medications and perform clinical procedures according to protocol
• Document patient encounters clearly and accurately in the medical record
• Collaborate with providers and behavioral health teams to coordinate care
• Educate patients and families on treatment plans, medications, and follow-up
• Support cross-coverage at other clinic locations as needed to maintain continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) credential in good standing
• Prior clinic experience required; outpatient, family practice, behavioral health, internal medicine or similar preferred
• Ability to work Monday-Friday daytime schedule and occasionally until 6:00 PM
• Willingness to assist at other clinic locations for coverage when needed
• Strong communication and interpersonal skills; patient-centered approach

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Solid knowledge of outpatient clinical workflows and triage
• Proficiency in medication administration and basic clinical procedures
• Comfortable with clinical documentation and electronic health records
• Ability to work collaboratively with multidisciplinary teams
• Good organizational skills and the ability to manage a busy clinic pace
