Roles & Responsibilities

• Provide high-quality, patient-centered physical therapy interventions to residents of the skilled nursing community
• Perform thorough assessments and develop individualized treatment plans that drive measurable progress
• Deliver therapeutic exercise, gait training, and functional mobility training to maximize independence
• Document clinical findings, treatment plans, and patient progress accurately and on time
• Collaborate with an interdisciplinary team including OT, SLP, nursing, and social services to optimize outcomes
• Educate patients and caregivers on home programs, safety strategies, and fall prevention
• Participate in team meetings and contribute to a positive, innovative therapy culture

Qualifications

• Current, unrestricted Physical Therapist license for New Mexico (must have or be eligible)
• Doctor of Physical Therapy (DPT) preferred; equivalent degree or education considered
• Current CPR/BLS certification
• Experience in skilled nursing, long term care, or post-acute rehab is preferred
• Strong clinical assessment and documentation skills with a patient-first mindset
• Excellent communication and teamwork skills to collaborate across disciplines

Education

• Doctor of Physical Therapy (DPT) or equivalent professional degree

Knowledge & Skills

• Patient assessment and treatment planning with a focus on functional outcomes
• Manual therapy, therapeutic exercise, and mobility/gait training techniques
• Knowledge of fall prevention, transfers, and adaptive equipment recommendations
• Proficiency with electronic medical record documentation (EMR)
• Strong time management and caseload organization skills
• Ability to coach and educate patients, families, and care staff effectively
