Roles & Responsibilities

Provide compassionate, high-quality direct patient care within scope of LPN practice
Administer medications and document administration accurately and timely
Monitor patient conditions, report changes to the interdisciplinary team, and escalate concerns
Assist with daily living activities and ensure patient comfort and safety
Collaborate with RNs, physicians, and support staff to implement care plans
Maintain accurate patient records and charting in the facility electronic system
Participate in infection control practices, safety protocols, and facility initiatives

Qualifications

Valid LPN license in Pennsylvania (required)
Graduate of an accredited LPN/LVN program
Current Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
Ability to pass background check, drug screen, and facility onboarding requirements
Strong communication skills, attention to detail, and a patient-centered approach
Prior long-term care or facility experience is a plus

Education

Graduate of an accredited LPN/LVN program (required)
High school diploma or GED (minimum)

Knowledge & Skills

Clinical skills in medication administration and basic nursing procedures
Familiarity with patient charting and documentation best practices
Ability to recognize and report changes in patient condition
Comfort working in a fast-paced, team-oriented healthcare environment
Good time management, organization, and empathy-driven care
