Roles & Responsibilities

Provide compassionate, high-quality practical nursing care to residents
Administer medications and treatments according to care plans and policies
Monitor patient status, document changes, and report concerns promptly
Collaborate with interdisciplinary team members to coordinate care
Assist with activities of daily living while promoting patient dignity
Maintain accurate, timely clinical documentation and compliance with facility standards

Qualifications

Active LPN license in Pennsylvania (or compact/state equivalent where applicable)
Completion of an accredited practical nursing program
Current BLS/CPR certification or willingness to obtain upon hire
Strong communication and patient advocacy skills
Comfortable working in a fast-paced long-term care or assisted living environment

Education

Graduation from an accredited LPN / Practical Nursing program
High school diploma or equivalent

Knowledge & Skills

Sound clinical assessment and basic nursing skills
Medication administration and documentation best practices
Infection control and safety procedures
Ability to use electronic health records and facility charting systems
Team collaboration and effective communication with patients and families
