Roles & Responsibilities

• Administer medications and treatments safely and accurately according to facility policy
• Monitor patient condition, record observations, and report changes to the care team
• Provide direct patient care including wound care, hygiene assistance, and mobility support
• Document care interventions clearly and timely in the patient record
• Collaborate with RNs, physicians, and interdisciplinary staff to deliver coordinated care
• Educate patients and families about care plans, medications, and follow-up needs
• Maintain a safe, clean, and compassionate care environment for residents

Qualifications

• Current LPN license in Pennsylvania (compact or state license as appropriate)
• Basic Life Support (BLS) certification or willingness to obtain
• Clinical experience caring for adults in a long-term care or skilled nursing setting preferred
• Strong communication skills and ability to work as part of a multidisciplinary team
• Current immunizations and ability to meet facility onboarding requirements

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program or equivalent clinical training

Knowledge & Skills

• Solid knowledge of medication administration and documentation practices
• Familiarity with infection control standards and safe patient handling
• Comfortable with electronic charting and clear written communication
• Ability to prioritize tasks and manage time during an 8-hour shift
• Empathy, patience, and strong interpersonal skills when interacting with residents and families
