Roles & Responsibilities

Provide direct, compassionate nursing care to residents and patients
Administer medications and treatments accurately and document thoroughly
Monitor vital signs and promptly report any changes to the care team
Assist with daily living activities and promote resident independence
Support RNs and the interdisciplinary team through clear communication
Maintain accurate charts, care plans, and shift handoffs
Follow infection control and safety protocols to protect residents and staff
Build positive relationships with residents and their families

Qualifications

Current Connecticut LPN license in good standing
Completion of an accredited Practical Nursing program
Current CPR/BLS certification or willingness to obtain
Able to pass background checks and required health screenings
Strong communication skills and a team-oriented attitude
Prior experience in long-term care or skilled nursing preferred

Education

Diploma or Certificate from an accredited Practical Nursing program

Knowledge & Skills

Medication administration and documentation
Basic nursing assessments and vital sign interpretation
Assistance with ADLs and patient hygiene
Electronic charting and basic computer skills
Wound care fundamentals and observation skills
Time management, prioritization, and teamwork
