Roles & Responsibilities

Provide compassionate, hands-on patient care and daily support to residents
Administer medications and treatments accurately and safely as directed
Monitor, record and report changes in patient condition, including vital signs
Collaborate with nurses, therapists and care team members to coordinate care plans
Maintain clear, timely documentation in patient charts and electronic records
Educate patients and families on care plans and procedures with empathy
Assist with admissions, discharges and safe patient transfers

Qualifications

Current LPN license (ability to practice as an LPN) — state licensure required
CPR/BLS certification (current or ability to obtain upon hire)
Previous hands-on clinical experience as an LPN is a plus
Strong communication skills and the ability to work well in a team-centered environment
Demonstrated reliability, attention to detail and commitment to quality patient care

Education

LPN / Vocational Nursing diploma or equivalent

Knowledge & Skills

Patient assessment and basic clinical procedures
Medication administration and accurate documentation
Familiarity with infection control and safety protocols
Comfortable working with electronic health records and charting
Excellent interpersonal skills and teamwork
