Roles & Responsibilities

• Provide compassionate, hands-on patient care and assist residents with activities of daily living
• Monitor and document vital signs, skin integrity, and behavioral changes to support the care team
• Assist with safe transfers, positioning, and mobility while following safety protocols
• Maintain accurate, timely documentation and communicate changes in condition to nursing staff
• Support infection control practices and maintain a clean, safe patient environment
• Collaborate with nurses, therapists, and families to promote individualized care plans
• Contribute to a positive patient experience by offering respectful, dignified care

Qualifications

• Active Licensed Nursing Assistant (LNA) certification — NH or VT licensure preferred or eligible
• Current CPR/BLS certification or ability to obtain prior to start
• Up-to-date immunizations and ability to pass required health screenings and background checks
• Comfortable working overnight shifts and performing physically active duties
• Strong communication skills and a team-first attitude
• Previous experience in long-term care or acute care settings is a plus

Education

• High school diploma or equivalent
• Completion of a state-approved LNA training program

Knowledge & Skills

• Hands-on knowledge of ADLs, transfers, and patient mobility techniques
• Basic vital signs assessment and documentation skills
• Familiarity with infection prevention and safety protocols
• Good charting and time-management abilities
• Compassionate bedside manner and effective communication with residents and families
• Ability to follow care plans and work collaboratively with clinical teams
