Roles & Responsibilities

• Provide compassionate, high-quality hospice nursing care during home and facility visits
• Perform skilled nursing assessments and develop individualized care plans
• Manage pain and symptoms to maximize patient comfort and dignity
• Support patients and families with education, counseling and bereavement resources
• Collaborate with interdisciplinary team members including social workers and volunteers
• Document patient care accurately and timely in the electronic medical record
• Participate in 24-hour on-call rotation and coordinate timely responses to patient needs
• Drive between sites safely and maintain professional communication with facility partners

Qualifications

• Current unencumbered Oregon Registered Nurse (RN) license required
• Healthcare Provider BLS certification required
• Valid driver's license and auto liability insurance required for patient visits
• Previous hospice, home health or case management experience preferred
• Broad-based nursing experience to support complex symptom management preferred
• Strong clinical judgment, empathy and excellent communication skills
• Ability to work independently in community and facility settings

Education

• BSN preferred; ADN or diploma with strong hospice/home health experience accepted

Knowledge & Skills

• Skilled in hospice nursing practices, pain and symptom management
• Experience with skilled nursing and home visits in community settings
• Comfortable providing family-centered education and bereavement support
• Strong assessment and clinical documentation skills
• Ability to coordinate care across facilities and community resources
• Familiarity with infection control and safety standards in home settings
• Reliable driving skills and knowledge of coastal geography for travel between sites
• Team-oriented mindset with excellent interpersonal skills

This contract places you along Oregon's beautiful central coast, working between Newport and Lincoln City. On your days off you can explore Yaquina Head Lighthouse and tidepools, visit the Oregon Coast Aquarium, stroll Newport's waterfront and historic bayfront, hunt for glass floats in Lincoln City, enjoy fresh seafood and local craft breweries, and take scenic drives to nearby coastal trails.
