Roles & Responsibilities

• Provide high-quality physical therapy care to long-term care and SNF residents, improving mobility and function
• Perform thorough patient evaluations and develop individualized treatment plans
• Deliver skilled interventions, progress patients toward goals, and document outcomes in Nethealth
• Coordinate care with nursing and interdisciplinary teams to ensure continuity and safety
• Manage caseloads across two facilities efficiently, providing one building per day coverage
• Communicate clearly with residents and families about goals, progress, and discharge planning

Qualifications

• Valid PT license in Texas (active and unrestricted)
• DPT or equivalent physical therapy degree (DPT listed as a primary skill)
• 1-3 years of clinical experience, ideally including long-term care/SNF experience
• Experience with Nethealth documentation or similar electronic medical record systems preferred
• Comfortable with multi-site coverage and frequent float between facilities

Education

• Doctor of Physical Therapy (DPT) or equivalent degree / current PT licensure

Knowledge & Skills

• Strong knowledge of long-term care and skilled nursing facility practices
• Proficiency in Nethealth or similar documentation systems
• Skilled in patient assessment, therapeutic interventions, and discharge planning
• Excellent communication and interdisciplinary collaboration skills
• Ability to manage time and caseload while providing compassionate care
