Roles & Responsibilities
- Provide high-quality physical therapy care to long-term care and SNF residents, improving mobility and function
- Perform thorough patient evaluations and develop individualized treatment plans
- Deliver skilled interventions, progress patients toward goals, and document outcomes in Nethealth
- Coordinate care with nursing and interdisciplinary teams to ensure continuity and safety
- Manage caseloads across two facilities efficiently, providing one building per day coverage
- Communicate clearly with residents and families about goals, progress, and discharge planning
Qualifications
- Valid PT license in Texas (active and unrestricted)
- DPT or equivalent physical therapy degree (DPT listed as a primary skill)
- 1-3 years of clinical experience, ideally including long-term care/SNF experience
- Experience with Nethealth documentation or similar electronic medical record systems preferred
- Comfortable with multi-site coverage and frequent float between facilities
Education
- Doctor of Physical Therapy (DPT) or equivalent degree / current PT licensure
Knowledge & Skills
- Strong knowledge of long-term care and skilled nursing facility practices
- Proficiency in Nethealth or similar documentation systems
- Skilled in patient assessment, therapeutic interventions, and discharge planning
- Excellent communication and interdisciplinary collaboration skills
- Ability to manage time and caseload while providing compassionate care
