Roles & Responsibilities

• Provide direct, hands-on patient care with compassion and attention to dignity
• Assist residents with activities of daily living including bathing, dressing, and mobility
• Monitor and record vital signs accurately and report changes to the nursing team
• Support safe transfers and fall prevention measures to protect residents
• Maintain clean, organized work areas and follow infection prevention guidelines
• Communicate clearly with nurses, families, and interdisciplinary team members
• Document care in the charting system accurately and in a timely manner

Qualifications

• Current CNA certification (CNA or T-CNA) required
• Current BLS/CPR certification or willingness to obtain prior to start
• Ability to pass background check, drug screen, and required health screenings
• Comfort working 12+ hour night shifts and the physical demands of resident care
• Previous long-term care or rehabilitation experience is a plus but not required

Education

• High school diploma or GED preferred
• Completed state-approved CNA program and current CNA certification

Knowledge & Skills

• Strong patient care and bedside skills tailored to long-term care and rehab
• Proficient with taking and documenting vital signs and basic assessments
• Knowledge of infection control, safe transfer techniques, and fall prevention
• Good communication and teamwork skills for a multidisciplinary care environment
• Ability to follow care plans and report changes in resident condition promptly
