Roles & Responsibilities
- Assess patients' functional abilities and needs to develop meaningful, patient-centered treatment plans
- Implement therapeutic interventions and adaptive techniques to improve daily living skills
- Document progress clearly and timely to ensure continuity of care and regulatory compliance
- Collaborate with interdisciplinary team members to coordinate care and maximize patient outcomes
- Educate patients and families on adaptive strategies, equipment usage, and home safety
- Advocate for patient needs and support smooth transitions across levels of care
Qualifications
- Active COTA (Certified Occupational Therapy Assistant) credential
- Current California OTA licensure or eligibility to obtain California licensure
- Comfort working in a post-acute/rehab setting and delivering hands-on patient care
- Strong communication and teamwork skills to collaborate with clinicians and families
- Willingness to complete facility onboarding requirements and maintain required immunizations
Education
- Associate degree or diploma from an accredited Occupational Therapy Assistant program
Knowledge & Skills
- Skilled in therapeutic interventions for ADLs, adaptive equipment, and mobility support
- Ability to perform patient assessments and document measurable goals
- Familiarity with post-acute rehabilitation workflows and interdisciplinary collaboration
- Strong clinical judgment, time management, and patient education abilities
- Comfortable with electronic documentation systems and charting
