Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with activities of daily living
• Monitor and document vital signs, intake/output, and changes in resident condition
• Support safe mobility and fall prevention through attentive assistance and proper technique
• Communicate clearly with the care team to ensure continuity and quality of care
• Maintain a clean, safe environment and follow infection control protocols
• Assist with feeding, bathing, dressing, and toileting while preserving resident dignity
• Participate in shift handoffs and accurate charting to support clinical decision-making

Qualifications

• Current CNA certification (required)
• T-CNA recognized and welcomed (training or certification a plus)
• BLS/CPR certification preferred or willingness to obtain upon hire
• Up-to-date immunizations and TB screening as required by facility
• Ability to pass background checks and drug screening
• Previous long-term care or skilled nursing experience is a plus but not mandatory

Education

• High school diploma or GED preferred
• Current CNA certification required

Knowledge & Skills

• Strong patient care skills with emphasis on safe transfers and mobility assistance
• Basic vital sign measurement and documentation proficiency
• Familiarity with infection control practices and PPE use
• Good communication and teamwork skills to collaborate with nurses and therapists
• Comfortable with electronic charting or willingness to learn facility EHR
