Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment for inpatient and outpatient populations
• Deliver acute care services including total joint management, med-surg, tele, and ICU support
• Treat orthopedic, neurologic, wound care, lymphedema, and pediatric patients as assigned
• Perform hands-on and home health therapy using OASIS and Cerner documentation where required
• Collaborate with PTAs, OT, SLP, rehab aides, nursing, and the interdisciplinary team for coordinated care
• Educate patients and families on treatment plans, discharge goals, and home programs
• Participate in unit-specific orientation and follow hospital policies and quality standards
• Adapt to a mix of inpatient, outpatient, and home health assignments based on facility need

Qualifications

• Active Physical Therapy license (able to submit) in applicable state
• Minimum 2 years of clinical PT experience (acute care, outpatient, or home health experience valued)
• BLS (Basic Life Support) certification required
• Experience with Cerner EMR required; OASIS experience required for home health roles
• Hands-on experience with total joints (knees, hips, shoulders), orthopedics, and basic neuro cases
• Comfort treating pediatric patients, wound care, and participating in lymphedema or hand therapy when assigned
• Strong documentation, communication, and teamwork skills

Knowledge & Skills

• Proficient in Cerner electronic charting
• OASIS documentation for home health settings
• Acute care assessment and interventions
• Orthopedics: total joints, shoulder, knee, spine care
• Neurologic rehab basics including stroke and TBI
• Wound care and lymphedema program familiarity
• Hand therapy experience for outpatient settings
• Patient and family education, discharge planning, and interdisciplinary collaboration
