Roles & Responsibilities
- Provide compassionate, patient-centered care following facility policies
- Administer medications and treatments accurately and document thoroughly
- Perform routine assessments and report changes in patient condition promptly
- Collaborate with nurses, therapists, and providers to support care plans
- Maintain accurate and timely documentation in the medical record
- Assist with activities of daily living to promote patient comfort and dignity
Qualifications
- Current Licensed Practical Nurse (LPN) license (Pennsylvania license or eligibility preferred)
- Completion of an accredited LPN/LVN program
- Basic Life Support (BLS/CPR) certification (or ability to obtain)
- Comfortable with medication administration and basic clinical procedures
- Strong communication skills and ability to work in a team
- Ability to pass required background checks and health screenings
Education
- Graduate of an accredited LPN/LVN program; high school diploma or equivalent
Knowledge & Skills
- Patient assessment and vital signs monitoring
- Medication administration and documentation
- Wound care and basic clinical procedures
- Familiarity with electronic health records and charting
- Infection control practices and patient safety principles
- Compassionate bedside manner and time management
