Roles & Responsibilities

Provide compassionate, hands-on care to residents including assistance with activities of daily living
Monitor and record vital signs and changes in resident condition, communicating concerns to the nursing team
Support mobility and transfer assistance while following safe patient handling protocols
Maintain a clean, safe and comfortable environment for residents
Assist with feeding, bathing, dressing and toileting using dignity-preserving techniques
Contribute to accurate documentation and timely handoffs to ensure continuity of care

Qualifications

Current CNA certification in Pennsylvania (required)
CPR/BLS certification preferred or available upon hire
Previous experience in long-term care or skilled nursing is a plus
Clear background check and up-to-date immunizations as required by facility
Reliable transportation to the Smethport location and a team-oriented attitude

Education

High school diploma or GED preferred
Current Certified Nursing Assistant (CNA) credential required

Knowledge & Skills

Strong resident-centered care and empathy
Basic clinical skills: vital signs, ADL assistance, safe transfers
Infection control and standard precautions knowledge
Good communication and teamwork with nurses and support staff
Basic charting and documentation skills
