Roles & Responsibilities

Provide compassionate, resident-centered nursing care and daily assessments
Administer medications and treatments safely and accurately
Monitor and document vital signs, changes in condition, and care provided
Collaborate with nurses, therapists, and interdisciplinary team members to optimize care plans
Perform wound care, IV support as applicable, and follow infection control protocols
Educate residents and families about care plans, medications, and health maintenance
Respond calmly and effectively to emergencies and escalate concerns appropriately

Qualifications

Valid LPN license in the state of Connecticut (must be active and in good standing)
Current BLS/CPR certification (or willingness to obtain upon hire)
Comfortable with medication administration and basic nursing procedures
Strong communication and teamwork skills with a focus on resident dignity
Prior long-term care or skilled nursing experience is a plus

Education

Graduation from an accredited Practical Nursing program (LPN diploma or equivalent)

Knowledge & Skills

Medication administration and documentation best practices
Basic assessment skills: vital signs, pain assessment, skin/wound observation
Familiarity with electronic health records and accurate charting
Infection prevention, safe patient handling, and fall prevention strategies
Ability to prioritize tasks in a fast-paced skilled nursing environment
