Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments safely and accurately
Perform routine assessments and monitor patient vitals
Document care thoroughly and maintain clear communication with the team
Support activities of daily living and promote patient dignity
Collaborate with nurses, LPNs, CNAs and interdisciplinary staff to coordinate care

Qualifications

Active Licensed Practical Nurse (LPN) license — ability to practice in Pennsylvania preferred
Current BLS/CPR certification or willingness to obtain
Comfortable with medication administration, charting, and basic assessments
Friendly, patient-centered communication skills and a team-first attitude
Ability to pass background check and any required health screenings

Education

Completion of an accredited LPN/LVN program (diploma or certificate)

Knowledge & Skills

Medication administration and documentation
Basic patient assessment and vital signs monitoring
Infection control and safety procedures
Clear charting and electronic health record (EHR) familiarity
Strong teamwork, time management, and patient-focused care
