Roles & Responsibilities

Provide compassionate, hands-on nursing care to assigned patients that improves outcomes
Perform assessments, record vital signs, and report changes promptly to the care team
Administer medications and treatments safely, following facility protocols
Maintain accurate patient documentation and update electronic health records
Collaborate with nurses, physicians, and allied staff to coordinate patient-focused care
Support patients and families with clear communication, education, and emotional support

Qualifications

Current LPN license; eligible to practice in Pennsylvania preferred
Current CPR/BLS certification or willingness to obtain upon hire
Recent clinical experience in long-term care or similar settings is a plus
Ability to pass background checks, drug screening, and required health screenings
Strong communication skills and a team-first attitude
Reliable, professional, and committed to high-quality patient care

Education

Graduate of an accredited practical nursing program (LPN diploma or equivalent)

Knowledge & Skills

Skilled at taking and interpreting vital signs and basic assessments
Medication administration knowledge with attention to safety and documentation
Comfortable with wound care, catheter care, and ADL assistance
Familiarity with infection control standards and safe clinical practices
Proficient in charting and using electronic health record systems
Excellent interpersonal skills for working with patients, families, and the interdisciplinary team
