Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents, ensuring safety and dignity
Administer medications and treatments accurately and in a timely manner
Perform thorough patient assessments and document findings clearly
Collaborate with nurses, therapists and care teams to develop and follow individualized care plans
Communicate professionally with residents, families and interdisciplinary teams
Maintain accurate and timely electronic medical records and shift handoffs

Qualifications

Active LPN license in Pennsylvania (required)
Completion of an accredited Practical Nursing program or equivalent
Current CPR/BLS certification (or willingness to obtain promptly)
Ability to pass background check and meet facility onboarding requirements
Compassionate bedside manner and strong communication skills

Education

Diploma or certificate from an accredited Practical Nursing/LPN program

Knowledge & Skills

Medication administration and documentation
Basic wound care and vital signs assessment
Familiarity with long-term care clinical workflows and resident-centered care
Comfortable working with electronic medical records (EMR/charting)
Team-oriented approach with good organizational skills
