Roles & Responsibilities

• Provide compassionate direct care to residents, promoting dignity and comfort
• Administer medications and treatments safely and accurately following facility policy
• Monitor resident status and report changes promptly to the care team
• Support activities of daily living including hygiene, mobility, and nutrition assistance
• Document care clearly and timely in the electronic medical record
• Collaborate with nurses, therapists, and providers to implement individualized care plans
• Participate in shift handoffs and communicate critical information to ensure continuity

Qualifications

• Current, active Massachusetts LPN license required; RN applicants with appropriate license are also considered
• Basic Life Support (BLS) certification required or obtained upon hire
• Previous experience in skilled nursing or long-term care preferred
• Strong clinical judgment, attention to detail, and a compassionate bedside manner
• Ability to pass facility background checks and meet onboarding requirements

Education

• Graduate of an accredited practical nursing program (LPN)
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and basic IV/med tech familiarity where applicable
• Wound care basics and pressure injury prevention
• Skilled nursing documentation and charting experience (EMR familiarity a plus)
• Infection control practices and safety-conscious care
• Clear communication with residents, families, and interdisciplinary teams
• Time management and prioritization during busy 12-hour shifts
