Roles & Responsibilities

• Coordinate and manage patient discharges to ensure safe, timely transitions of care
• Perform utilization review and apply evidence-based criteria to determine medical necessity
• Assess patient needs and arrange DME and community resources as part of discharge planning
• Collaborate with interdisciplinary team members, including social services, to optimize patient outcomes
• Document case management activities in Epic and maintain accurate, timely charts
• Apply admission criteria and InterQual guidelines where appropriate to support utilization decisions
• Advocate for patients while ensuring regulatory compliance with HIPAA and CMS requirements

Qualifications

• BSN required
• Active CA RN license required
• AHA BLS required
• Minimum 2 years clinical hospital RN experience; hospital-based case management experience (2–3 years preferred)
• Travel experience required — no first-time travelers
• Epic charting experience preferred
• Familiarity with InterQual, utilization review, discharge planning, and medical necessity determinations
• Ability to work business hours, attend orientation (72 hours), and collaborate in a fast-paced acute hospital
