Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (30-minute lunch) Day; no call; every-other-weekend coverage as noted
Hours per Week: 40
Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Coordinate and manage patient discharges to ensure safe, timely transitions of care
- Perform utilization review and apply evidence-based criteria to determine medical necessity
- Assess patient needs and arrange DME and community resources as part of discharge planning
- Collaborate with interdisciplinary team members, including social services, to optimize patient outcomes
- Document case management activities in Epic and maintain accurate, timely charts
- Apply admission criteria and InterQual guidelines where appropriate to support utilization decisions
- Advocate for patients while ensuring regulatory compliance with HIPAA and CMS requirements
Qualifications
- BSN required
- Active CA RN license required
- AHA BLS required
- Minimum 2 years clinical hospital RN experience; hospital-based case management experience (2–3 years preferred)
- Travel experience required — no first-time travelers
- Epic charting experience preferred
- Familiarity with InterQual, utilization review, discharge planning, and medical necessity determinations
- Ability to work business hours, attend orientation (72 hours), and collaborate in a fast-paced acute hospital

- Determining medical necessity using evidence-based guidelines
- Admission criteria and needs assessment/DME ordering
- Epic charting and electronic documentation best practices
- Understanding of HIPAA and CMS regulatory standards
- Strong interdisciplinary communication and patient advocacy skills
