Roles & Responsibilities
- Provide safe, compassionate patient care during assigned evening shifts
- Administer medications and treatments accurately and document carefully
- Perform thorough patient assessments and monitor changes in condition
- Collaborate with RNs, physicians and interdisciplinary team members
- Educate patients and families about care plans, medications and follow-up
- Maintain clear, timely electronic medical record documentation
- Adhere to infection control, safety and regulatory standards
- Support admissions, transfers and discharges to ensure smooth patient flow
Qualifications
- Current, unrestricted Pennsylvania LPN license (required)
- BLS/CPR certification (American Heart Association or equivalent) required
- Completion of an accredited practical nursing program
- Clinical experience in acute care or long-term care preferred
- Up-to-date immunizations and ability to pass background/drug screening
- Strong communication skills and ability to work well in a team
Education
- Graduate of an accredited practical nursing (LPN) program
- High school diploma or GED required
Knowledge & Skills
- Medication administration and documentation best practices
- Patient assessment, vital signs and basic clinical procedures
- Wound care fundamentals and basic dressing changes
- Comfortable with electronic medical records and charting
- Time management and prioritization in a fast-paced setting
- Ability to follow care plans and escalate changes appropriately
