Roles & Responsibilities
- Provide compassionate, safe patient care during overnight shifts, including assessments and interventions
- Administer medications and treatments accurately and document care in the electronic health record
- Monitor residents for changes in condition and escalate concerns to the interdisciplinary team
- Collaborate with nursing assistants, therapists, and providers to create and execute individualized care plans
- Maintain a safe, clean environment and follow infection control protocols
- Communicate clearly and empathetically with patients and families to support quality of life
- Participate in shift handoffs and ensure thorough documentation for continuity of care
Qualifications
- Current, unrestricted LPN or RN license (able to work as either LPN or RN per posting)
- Basic Life Support (BLS) certification required; ACLS or PALS as applicable or preferred
- Experience in long-term care, skilled nursing, or geriatric nursing is highly desired
- Ability to work overnight shifts and weekends (Sunday, Friday, Saturday)
- Current immunizations, TB test, and successful background & drug screening per facility policy
- Strong communication skills and a team-first attitude
Education
- Graduate of an accredited LPN or RN nursing program (diploma, associate, or BSN as applicable)
- Additional certifications or continuing education in geriatric/long-term care encouraged
Knowledge & Skills
- Clinical assessment and basic nursing procedures relevant to LPN/RN scope of practice
- Med administration, wound care, and monitoring chronic conditions common in long-term care
- Proficient documentation in EHRs and accurate charting practices
- Time management and prioritization for overnight workloads
- Infection control practices and safe patient handling
- Collaborative teamwork and clear communication with interdisciplinary staff
