Roles & Responsibilities
- Provide direct patient care delivering high-quality speech therapy services in patients' homes
- Perform thorough patient assessments and create individualized care plans that make a measurable difference
- Collaborate with interdisciplinary team members to coordinate and optimize patient outcomes
- Document clinical findings, progress, and treatment plans clearly and promptly
- Use and instruct patients/caregivers on communication aids, adaptive devices and rehabilitation equipment
- Work independently with minimal supervision to schedule visits and manage caseload effectively
- Participate in professional development and contribute to a culture of safety and excellence
Qualifications
- Massachusetts Speech-Language Pathologist license (required)
- Certificate of Clinical Competence (CCC) — Clinical Fellow candidates accepted
- Master's or Doctorate in Speech-Language Pathology (required)
- Minimum 2 years SLP home care experience (required)
- Active American Heart Association BLS (AHA) certification (required)
- Valid driver license and reliable own vehicle (required)
- Ability to work independently and manage a home health caseload
Education
- Master's or Doctorate in Speech-Language Pathology (required)
Knowledge & Skills
- Direct patient care and patient-centered assessment skills
- Care plan development and progress monitoring
- Interdisciplinary collaboration and communication
- Familiarity with rehabilitation equipment, language/communication aids and assistive devices
- Experience with acute care transitions is a plus
- Comfort working autonomously in patients' homes and adapting interventions to home environments
- Strong documentation and time-management skills
