Roles & Responsibilities
- Provide compassionate, competent nursing care to patients as an LPN
- Administer medications and treatments accurately and document actions
- Monitor and report changes in patient condition to the care team
- Assist with wound care, vital signs, and routine clinical procedures
- Collaborate with RNs, physicians, and interdisciplinary staff to coordinate care
- Educate patients and families on care plans and safe home practices
- Maintain accurate, timely documentation in the patient record
- Follow infection control, safety, and facility policies to protect patients and staff
Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- Current BLS/CPR certification or willingness to obtain prior to start
- Ability to pass background check and any required health screenings
- Comfortable working in a fast-paced clinical setting and adapting to patient needs
- Strong communication skills and a team-first attitude
- Previous LPN experience in long-term care or similar settings is a plus
Education
- Graduation from an accredited LPN program or equivalent practical nursing diploma
Knowledge & Skills
- Clinical assessment and basic nursing procedures
- Medication administration and documentation best practices
- Familiarity with electronic health records and accurate charting
- Infection control principles and patient safety focus
- Time management and prioritization during 12+ hour shifts
- Empathy, clear communication, and teamwork
