Roles & Responsibilities
- Deliver compassionate direct patient care, including medication administration and wound care
- Perform timely assessments, monitor vital signs, and report changes to the care team
- Document patient status and interventions accurately in the medical record
- Collaborate with RNs, providers, and support staff to create and follow individualized care plans
- Assist with activities of daily living while preserving patient dignity and promoting independence
- Participate in shift handoffs to ensure seamless continuity of care
Qualifications
- Licensed Practical Nurse (LPN) — active and in good standing (required)
- Current basic life support (BLS) certification preferred or willing to obtain
- Comfort working in a long-term or specialty care setting such as Hopkins Center
- Strong communication skills and a patient-first attitude
- Ability to follow facility policies, maintain accurate charts, and collaborate with a multidisciplinary team
Education
- Graduate of an accredited Practical Nursing (LPN) program; diploma or certificate required
Knowledge & Skills
- Proficient with medication administration and documentation
- Skilled in taking and interpreting vital signs and basic patient assessments
- Familiarity with electronic health record charting (or quick to learn)
- Good clinical judgment, time management, and teamwork
- Strong interpersonal skills for patient and family interactions
