Roles & Responsibilities
- Assess patients’ functional abilities and develop individualized OT plans
- Implement therapeutic interventions to improve independence with ADLs
- Collaborate with nursing, PT, SLP and interdisciplinary team members
- Document progress and outcomes clearly and efficiently in the EMR
- Provide caregiver and family education to support safe discharge
- Adapt treatment plans based on patient response and facility needs
Qualifications
- Active or eligible Occupational Therapy license for New Mexico
- New graduates welcome — experience in SNF settings is a plus
- Current BLS/CPR certification preferred or willing to obtain
- Strong communication and teamwork skills for interdisciplinary care
- Ability to manage a varied caseload and adapt to facility routines
Education
- Degree in Occupational Therapy (Master’s or OTD preferred) — entry-level/new grads welcome
Knowledge & Skills
- Skilled in OT evaluations, ADL retraining, and adaptive equipment recommendations
- Familiarity with Skilled Nursing Facility (SNF) workflows and documentation
- Good clinical reasoning for geriatric and post-acute patient populations
- Comfortable with common therapeutic modalities and activity-based interventions
- Efficient charting and outcome-driven treatment planning
