Roles & Responsibilities
- Provide patient-centered physical therapy interventions in a skilled nursing facility setting
- Collaborate with rehab team members to develop and implement individualized treatment plans
- Perform thorough documentation of patient progress, outcomes, and plan of care
- Educate patients and families on therapeutic exercises, safety, and functional goals
- Communicate clearly with nursing and facility staff to coordinate patient care
- Monitor and modify treatments based on patient response and clinical judgment
- Support discharge planning and transition of care with timely communication
Qualifications
- Credentialed as a Physical Therapist Assistant (PTA) or eligible for state licensure
- Comfortable working in a Skilled Nursing Facility practice setting (SNF)
- Open to all experience levels including new grads; positive attitude and eagerness to learn
- Strong interpersonal and communication skills for team-based care
- Ability to complete required documentation in a timely, accurate manner
- Current CPR/BLS certification or willingness to obtain prior to start (facility dependent)
Education
- Associate degree in Physical Therapist Assisting or related accredited PTA program
- New graduates are welcome to apply
Knowledge & Skills
- Fundamental PTA clinical skills: therapeutic exercise, mobility training, transfers and gait training
- Knowledge of SNF workflows, Medicare/Medicaid basics, and documentation expectations
- Skilled in patient assessment, goal setting, and outcome measurement
- Comfortable collaborating with interdisciplinary teams and family caregivers
- Time management skills to balance caseload and documentation requirements
