Roles & Responsibilities
- Provide skilled occupational therapy interventions as a Certified Occupational Therapy Assistant (COTA) in a skilled nursing facility
- Implement individualized treatment plans to help residents regain independence and function
- Collaborate closely with the therapy team and interdisciplinary staff to ensure coordinated care
- Document treatment sessions and patient progress accurately and promptly according to CRG policies
- Meet facility productivity expectations (92%) while maintaining quality, patient-centered care
- Adhere to facility dress code and on-site presence requirements to receive guaranteed hours
- Communicate with patients and families to set goals and support meaningful outcomes
Qualifications
- Current COTA credential (Certified Occupational Therapy Assistant)
- 1-3 years of experience preferred, ideally with SNF/skilled nursing experience
- Eligible for or holding state OTA/COTA licensure as required by New Mexico
- Ability to follow CRG policies & procedures and meet productivity standards
- Strong communication and teamwork skills; reliable on-site presence
Education
- Graduate of an accredited Occupational Therapy Assistant (OTA) program / COTA certification
Knowledge & Skills
- Skilled nursing facility (SNF) rehabilitation practices and common geriatric diagnoses
- Therapeutic activity planning and adaptive equipment use
- Accurate clinical documentation and time management to meet productivity goals
- Patient-centered care, safety precautions, and infection control
- Collaborative care with PT, SLP, nursing, and facility staff
- Professional presentation and adherence to facility dress code
