Roles & Responsibilities
- Provide safe, compassionate direct patient care as part of the interdisciplinary team
- Administer medications and treatments accurately and document according to facility policy
- Conduct focused assessments and report changes in patient status promptly
- Assist with care planning and collaborate with RNs and providers to achieve patient goals
- Maintain clear, timely charting and ensure regulatory documentation is complete
- Support infection control practices and promote a clean, safe environment
- Deliver patient and family education in a clear and caring manner
Qualifications
- Active LPN license or eligible for licensure in New Mexico
- Current Basic Life Support (BLS) certification
- Recent clinical experience as an LPN, ideally in long-term care or similar settings
- Strong communication skills and a collaborative attitude
- Up-to-date immunizations and ability to meet facility onboarding requirements
Education
- Graduate of an accredited Practical Nursing program or equivalent
- High school diploma or GED required
Knowledge & Skills
- Proficient in medication administration, wound care basics, and vital sign monitoring
- Comfortable with electronic health record documentation and standard charting
- Knowledge of infection prevention and patient safety practices
- Ability to prioritize tasks in a fast-paced, dynamic clinical environment
- Compassionate bedside manner and strong patient advocacy
