Roles & Responsibilities
- Provide compassionate, hands-on nursing care: administer medications, monitor vitals, and document care accurately
- Collaborate with RNs, physicians, and the interdisciplinary team to implement and update care plans
- Assist residents with activities of daily living while promoting independence and dignity
- Recognize changes in patient condition and escalate concerns promptly to keep residents safe
- Maintain timely, accurate EMR/charting and adhere to infection control and facility policies
- Support smooth admissions, discharges, and care transitions to ensure continuity of care
Qualifications
- Active LPN (Licensed Practical Nurse) license in good standing; travel licensure or ability to obtain is a plus
- T-LPN / Travel LPN experience preferred
- Current BLS/CPR certification required or must obtain upon hire
- Comfortable working in long day shifts and managing a diverse patient load
- Strong communication skills and the ability to work well within a multidisciplinary team
- Up-to-date immunizations and any facility-required screenings
Education
- Graduate of an accredited practical nursing (LPN/LVN) program or equivalent
Knowledge & Skills
- Medication administration and accurate documentation
- Patient assessment and vital sign monitoring
- Basic wound care and catheter care
- Proficiency with electronic medical records and charting
- Infection control best practices
- Time management and the ability to prioritize in a busy clinical environment
